A template helps you keep your own copy tidy. It does not replace the clinic's record, which carries the signature or stamp that daycares and authorities ask for.

The profile page
The profile holds the facts that rarely change: name, birthday, breed, sex, color, microchip number, your contact details and the vet's details. Add allergies, conditions and medications as your vet described them.

The vaccine table
Make one row per vaccine given. Columns: vaccine name, date given, valid until or next due, clinic. Copy each value from the clinic's record. Do not guess a due date.

Attach a photo of the original page next to the table so you can prove each row.
The medication list
List each medication with the name as printed on the label, the dose and times as written by your vet, the start date and the end date. If it is ongoing, note the next refill.
This list is for you and for anyone who helps. It records what you were told and does not suggest anything. Our medication reminder guide covers setting nudges.
If something changes, add a new row with the date instead of overwriting the old one.
The visit log
Record each vet visit: date, clinic, reason in your own words, what the vet said to do and any cost. A line or two is enough. The log lets you answer 'when did we last see someone about that?' in seconds.

Keep invoices with the log. They help with insurance.
The weight log
Write the date and the number each time your dog is weighed. The clinic often weighs at visits. A home scale can fill the gaps if your vet asks for it. Keep the unit consistent.

Ask your vet what weight range is right for your dog and write down the answer with the date.
How do I keep the record on my phone?
A spreadsheet, a notes document or an app all work. What matters is that the record has dates, that you update it after each visit and that you can find it in a minute.

How do I share it safely?
Share only what the recipient needs. A daycare needs the vaccine page. A sitter needs the routine and contacts. A new vet needs the whole history. Send PDFs rather than editable files.

Never post health records publicly.
How often should I update the record?
Update it the same day as every vet visit, every new medication and every new vaccine. Five minutes after an appointment, while the details are fresh, is far easier than an hour of reconstruction before a trip. Set a monthly reminder if you tend to forget.
Once a year, review the whole record. Check that the profile is current, remove old medications and confirm that the contact numbers still work.
Mark the review date in your calendar so it does not slip.
What should I do when the clinic changes?
Ask the old clinic to send your dog's full history to the new one, and request a copy for yourself. Compare it with your own template, and ask the new clinic to fix any difference. Records can differ between clinics, and your own copy helps you notice.
Update the vet contact details in your profile and in any shared sheets. Our moving checklist covers the full transfer.
Keep the old clinic's number as well, since you may need to reach them for older records.
What about records for a senior dog or one with ongoing needs?
The same template works, with a longer medication list and a visit log you update more often. Keep the most recent entries at the top, so the person reading sees the current picture first.
Bring the record to every visit. A vet who sees a clear timeline can focus on your dog and not on rebuilding history. If you are worried about your dog's health at any time, contact your veterinarian.

What this template is not
